Post-Acute Care

Skilled and sub-acute nursing facilities provide the highest level of care to a patient outside of the hospital. They may provide short-term, rehabilitation, or long term care. Skilled and sub-acute nursing facilities provide a room, meals, skilled nursing and rehabilitative care, and medical services in a safe, supervised environment.

Skilled and sub-acute facilities are certified by the state and federal government agencies to provide levels of care that range from custodial care to skilled nursing that can only be delivered by trained professionals.

In 2016, Affinia Health Network (now Trinity Health Alliance of Michigan) began the Skilled Nursing Facility Preferred Partner Collaboration. Since that time, our criteria and the facilities participating in the High Performing Network have evolved.

In 2017, St. Joseph Mercy Health Partners Clinically Integrated Network (CIN) developed a High Performing Providers Network of Skilled Nursing Facilities for our Washtenaw/Livingston/Oakland and Wayne Market.

West Michigan:

Regional Criteria for 1st Tier High Performing Skilled Nursing Facilities includes the following:

  • Attendance at monthly Post-Acute Town Hall Meetings
  • CMS 5-STAR quality rating of 3 or higher
  • Participation in clinical programs such as Congestive Heart Failure and COPD (starting 2023)

Regional Criteria for 2nd Tier High Performing Skilled Nursing Facilities include the following:

  • Attendance at monthly Post-Acute Town Hall Meetings
  • Recommended CMS 5-STAR quality rating of 3 or higher for 7 out of the last 12 months
  • Participation in clinical programs such as Congestive Heart Failure (CHF) and COPD (starting 2023)

Southeast:

A Regional Criteria for High Performing Skilled Nursing Facilities includes the following: Attendance at quarterly regional meetings, Quarterly data submission with the focus on quality metrics, CMS 5-Star quality rating of 3 or higher and participation in hospital based programs such as SEPSIS, Stroke and Heart Failure.

High Performing Status indicates that a Skilled Nursing Facility is meeting all of our requirements, and is or will be listed as a high performing SNF provider in our network’s High Performing Skilled Nursing Facility Provider List.

We review our criteria on a quarterly basis and update our list of a high performing Skilled Nursing Facilities accordingly.
